The following information has been compiled to assist our new patients and their families in preparing for a procedure at Children's Surgery Center. If there is any additional information you may need, or if there is anything that may need further explanation, please do not hesitate to contact us
Medical History
To be completed by your child's primary care doctor. Please also note that the History and physical form must be completed within 30 days of your child's scheduled procedure.
Preadmission Medical AssessmentForms
If you are not using the Medical Passport please read our forms section and bring the "Receipt and acknowledgment" form with you to your appointment.
